Kisspeptin is a neuropeptide encoded by the *KISS1* gene. It sits upstream of nearly every signal that drives human reproduction — telling GnRH neurons when and how often to fire. Without it, puberty doesn’t happen. People with loss-of-function mutations in the kisspeptin receptor never enter puberty without exogenous hormone replacement, a finding that rearranged reproductive endocrinology when it was identified in 2003.

What makes kisspeptin different from other reproductive peptides is positional. GnRH executes the signal; kisspeptin tells GnRH when to fire. It binds KISS1R, a G-protein-coupled receptor on GnRH neurons, triggering a calcium cascade that depolarizes the cell. The neuron fires, GnRH gets released, the pituitary responds with LH and FSH, and the gonads produce testosterone, estradiol, and gametes. The biology is unusually clean for a neuropeptide. The clinical picture is messier — kisspeptin has real human trial data, a clear mechanism, and zero approved products.
